INFORMATION FOR LICENSURE APPLICATIONS

PLEASE NOTE: PROPER DESIGNATION OF YOUR STATE LICENSURE IS LISTING YOUR STATE LICENSURE FOLLOWED BY YOUR NATIONAL CERTIFICATION: example: John Doe, LAT, ATC

 

Please Note: NC does offer any type of temporary or provisional license.  

 

Register at the LAT Portal to begin your licensure application.
The NCBATE will review COMPLETE applications only around the 1st and 15th of each month.

Fees associated with licensure include:
1.  $200 Licensure Fee
2.  $75 Renewal Fee
3. $100 Late Renewal Fee (if paid after Jan 31)
     (AUTHORITY:  NC Administrative Code 21 NCAC 03 .0201 FEES)
 

If your licensure has expired and you would like to reinstate it, you can find your reinstatement application under your profile on the LAT PORTAL

TROUBLESHOOT TIP:  If you find yourself at a point in your application where you cannot move forward but the application seems to be complete make sure the protocol tool bar at the top of the application is complete.  If it is not, you are missing a required upload of a protocol.  
If you are unemployed in NC, and seeking licensure you may still apply for licensure by following these steps: Select “Add Protocol” in the Protocol Section of the application then select “unemployed” in the dialog box.  This will complete your protocol requirement in the application. **NOTE:  You may not practice athletic training in NC until you have a protocol filed with the NCBATE.  Protocols can be filed and updated at any time.  

Reinstatement Requirements (If your LAT is expired):

1. Completed LAT PORTAL REINSTATEMENT APPLICATION and CEUs (see hourly requirements below)
2. $100 Reinstatement Fee
3. Upload of current BOC card or verification from BOC website of current BOC certification
4. Upload of Protocol

The CEU requirements for reinstatement are as follows:

21 NCAC 03 .0303 LAPSED LICENSE
(a) The lapsed license may be renewed within a period of five years after expiration upon payment of the lapsed license fee and the completion of 25 contact hours of continuing education for each year that the license has lapsed.

(b) A license that has lapsed for more than five years shall be renewed upon payment of the lapsed license fee and the completion of 28 hours of continuing education for each year that the license has lapsed.
